The Core of Asthma Management: Corticosteroids

Corticosteroids:

  • Modulate inflammatory response
  • Onset of anti-inflammatory effect is delayed until 4-8hrs after administration
  • When given within 1 hour of ED arrival, shown to decrease need for hospitalization
  • PO and IV have been shown to have same efficacy
  • Discharge pts on 5-10 day non-tapered, steroid burst or can give IM depot of methylprednisolone
  • Inhaled corticosteroids only have a role in maintenance therapy rather than in acute exacerbations
